Our care coordinators work with particular groups of patients in our surgeries and run our Enhanced Health Checks. Alongside our GPs and other clinical staff, they ensure that support is available to patients and their carers. They may also be involved in supporting patients in care homes and supporting patients in preparing for or following up conversations with clinicians about their treatment.
